Sliding Scale Fee Program

As a Federally Qualified Health Center, which is non-profit and partially federally funded, Grand Peaks is able to offer services on a sliding scale to patients who qualify. Your modified fee depends on income and household size. Once the reduced rate is determined, your patient responsibility may be significantly less. The graphic below illustrates an example of an appointment with the sliding scale applied.
