Patient Portal

Grand Peaks is pleased to offer an electronic version of your records via the Patient Portal! The Patient Portal is a tool which allows you to view your medical and dental information online in a secure website or on the “Healow” app via your smart-phone, tablet, or other electronic devices.

Patient Portal Options

  • Demographic Information Update your basic information to keep your account current
  • Appointments Your appointment dates as well as a summary of your care at the appointment
  • Messages Send a message to our staff who will directly consult our Providers on your behalf
  • Medical Records Your personal health record
  • Lab Results View laboratory results as they are completed
  • Immunizations Immunizations you have received
  • Patient Education Access educational materials & research other topics
  • Reminders Receive health related reminder messages
  • Billing View your statement or print out as needed for tax or reimbursement purposes

How do I get started?

Provide us with an email address and your patient account will automatically be Patient Portal enabled.” *Please contact us if your email address changes, if you wish to discontinue use of the Patient Portal, or have questions.*

You will receive an email with a User Name and a Password.

To access the Patient Portal, use one of these options:

  • Click on the link in the email
  • Visit our website: grandpeaks.org and choose the Patient Portal button (top right)
  • Use the “Healow” and/or the “Healow Mom” app on your smart phone, tablet, or other electronic device.

Login with the Username and Password and answer a question related to your account.

Choose a new password and security question.
