As a non-profit Community Health Center, the Grand Peaks organization is committed to providing affordable, quality medical, dental and Behavioral healthcare to our patients. Whether you are insured, self-pay, uninsured, or low-income, we offer affordable rates. These rates are discounted on a sliding-fee scale based on documentation of patient/family income and household size.
Government, insurance and coverage programs that we accept and bill include:
Government Programs: Medicare, Medicaid, CHIP, VA Choice Program, Tricare, TriWest
Commercial Insurance: Blue Cross, Regence Blue Shield, Pacificsource, DMBA, Select Health, Aetna, Mountain Health Co-op, many United Healthcare plans, as well as companies covered by the IPN, and the First Choice Health network.

Our organization has adopted the Patient Centered Medical Home (PCMH) model of patient care. This model of care uses a team-based approach to provide patients with efficient and comprehensive healthcare. Grand Peaks has received Level 3 Recognition by the National Committee for Quality Assurance at its St. Anthony Medical location. Recognition of this type demonstrates our commitment to providing the best possible healthcare to you and your family.
