Grand Peaks is a Federally Qualified Health Center (FQHC) that began as a small medical clinic in 2007, with one Provider. In 2010 a Dental Clinic was added in St. Anthony. In 2015 another Medical Clinic location was added in Rexburg with a Dental Clinic added the following year, spring 2016. In 2017, the Behavioral Mental Health services in were added as a new building in St. Anthony. In spring 2020, Behavioral Health was also added at the Rexburg location with a large renovation. We have grown in size and services to serve patients in St. Anthony, Rexburg and communities in the Upper Valley and surrounding areas.
As a non-profit Community Health Center, Grand Peaks is committed to providing affordable, quality medical, dental and behavioral health services to our patients. Whether you are insured, self-pay, uninsured, or low-income, we offer affordable rates. These rates may also be discounted on a sliding-fee scale, which is based on documentation of patient/family household size and income.
