About Us
Grand Peaks is a Federally Qualified Health Center (FQHC) that began as a small medical clinic in the Upper Valley in 2007. In 2009 we expanded our services to include a dental clinic in St. Anthony. Medical services were added in Rexburg in 2015 followed by dental services a year later also in Rexburg. Expansion continued and in the summer of 2017 Behavioral Health services were added in a newly constructed building on our St. Anthony campus. In the spring of 2020, Behavioral Health services were also added at the Rexburg location with the conclusion of a large renovation. With continued growth came a need for a new medical building in St. Anthony, which was completed and opened in the summer of 2020. This new building facilitated the addition of a full service pharmacy.
As a non-profit Community Health Center, Grand Peaks is committed to providing affordable, quality medical, dental, behavioral health and pharmacy services to our patients. Whether they are insured, self-pay, under-insured, or low-income, we offer affordable rates. These rates may also be discounted on a sliding-fee scale, which is based on documentation of patient/family household size and income.

To earn this badge, a clinic must either increase the number of people they serve, improve patient care quality, or expand services like mental health, dental, or vision care.
This badge demonstrates that our team at Grand Peaks is not only committed to delivering high-quality care but also continuously broadening access to essential health services for our community. This attitude perfectly aligns with our mission statement.
Improving Health Outcomes for Underserved Communities
Grand Peaks Medical has been awarded the 2024 HRSA Health Disparities Reducer Badge, which recognizes our efforts to address and reduce health disparities among various racial and ethnic groups.
To get this badge, a health center must first qualify for the Access Enhancer Badge and then show improvement in important areas like controlling high blood pressure, managing diabetes, or reducing low birth weight rates. These improvements must specifically benefit underserved racial or ethnic groups.

Leveraging Technology for Better Care
Grand Peaks Medical has been honored with the 2024 Advancing Health Information Technology (HIT) for Quality Badge, which acknowledges our efforts to use technology to improve patient care.
To earn this badge, a health center must have an electronic health record (EHR) system in place, offer telehealth services, securely share patient information with other providers, and use technology to engage patients in their care. Grand Peaks Medical also collects important data on patients’ social risk factors to ensure they receive the best care possible.
This badge shows that we are committed to staying at the forefront of healthcare technology to benefit our patients.

This is to notify you that under Federal law relating to the operation of community health centers, the Federal Tort Claims Act (FTCA), (See 28 U.S.C. §§ 1346 (b), 2671-80), provides the exclusive remedy for damage from personal injury, including death, resulting from performance of medical, surgical, dental, or related functions by any Upper Valley Community Health Services, DBA Grand Peaks Medical, Dental or Behavioral Health practitioner, who the Department of Health and Human Services has deemed to be an employee of the Public Health Service. This FTCA Medical Malpractice coverage applies to deemed Healthcare Practitioners who have provided a required or authorized service under Title XIX of the Social Security Act (i.e. Medicaid Program) at Upper Valley Community Health Services, Inc. DBA Grand Peaks Medical, Dental & Behavioral Health site or through offsite programs or events carried out by Upper Valley Community Health Services, DBA Grand Peaks Medical, Dental & Behavioral Health (see 42 U.S.C. § 233(a), (o)).
