Jacob Moss, MD, JD

Dr. Jacob Moss joined the Grand Peaks Medical team in September 2025. Prior to joining the staff, Dr. Moss provided primary care for patients at Teton Valley Hospital in Driggs, Idaho. During his time there, he also served as the outpatient clinical director, a member of the medical executive committee, and assistant chief of staff. He is experienced in managing a broad array of acute and chronic medical conditions with special interests in mental health and addiction medicine.

As a Rexburg native, Dr. Moss has long aspired to provide medical care for the residents of southeast Idaho, and he is excited to give back to the people and communities that have provided so much for him and his family over the years. He and his beautiful wife, Catherine, reside in Rexburg and have three amazing children. They love spending time outdoors and especially enjoy flying, fishing, and snowboarding together.
