Congratulations to Grand Peaks Medical and Dental for being chosen by the Board of Directors as Business of the Month for November!
As a non-profit Community Health Center, Grand Peaks is committed to providing affordable, quality medical, dental, and behavioral healthcare to our patients.
Grand Peaks is a Federally Qualified Health Center (FQHC) that began as a small medical clinic in 2007, with one Provider. In 2010, a Dental Clinic and staff were added in St. Anthony. In 2015, an additional Medical Clinic service location was added in Rexburg. The following year the Dental Clinic was added. In 2017, the Behavioral and Mental Health Clinic was built in St. Anthony. Coming in Spring 2020, the Medical clinic in St. Anthony will be moving to a newly constructed building and Behavioral Health serviced will be added in Rexburg.
We have grown in size and services will be added in Rexburg. Whether you are insured, self-pay, uninsured, or low-income, we offer affordable rates. These rates may be discounted on a sliding fee scale, to those who qualify, based on proof of patient/family household size and income.
